How Homecare Works

Home care is the provision of a range of care and support services delivered to a person, including older people, in their own home. The following steps are involved in arranging for homecare for yourself or a loved one:

Step 1

Get in contact with us either by our online enquiry form and we will call you back or by phone on 01 230 0020.

Step 2

We discuss your needs by phone. We know from experience that the best first step is a conversation about your specific situation.

Step 3

We carry out a Care Consultation with you. This is a no-obligation, completely free visit, during which our Nurse Manager will offer advice and resource information and assess risks to health and wellbeing. The Care Consultation normally takes place in the home, or in the hospital if this is relevant. Either way we encourage family members to attend and contribute to this stage where possible.

Step 4

We develop a Care Plan for your approval. The Care Plan includes carer responsibilities, proposed weekly schedule and potential cost. The Care Plan is tailored and timed to introduce homecare to the individual client in the most appropriate and effective way.

Step 5

We introduce you to the carer we have selected for you, once you have decided to proceed. We will have given a lot of thought to the most suitable carer for you or your loved one, taking into account the client’s likes and personality, interests and hobbies. But if the “match” isn’t right we will immediately move to select an alternative carer. Our team begins caring for your loved one.

Once the homecare service is up and running, you can rely on our excellent team to reliably inform you of any changes that might affect the service.

Also, our Quality Manager will visit the home regularly to make sure everything is happening as it should, and to re-assess the client’s needs.

Alzheimer’s / Dementia Care

At Heritage Homecare we can provide Carers who are specifically trained in expert, person-centred Alzheimer’s and Dementia care.

Convalescent Care

Although you or your loved one will probably be looking forward to getting home, you may also have some worries about how they will cope when they get there.

Live-in Care

We provide specialist Live-in Homecare when it is agreed to be the best solution for the client and their family.

Overnight Care

We provide the security of an overnight presence in the home, for peace of mind for those living alone, or sometimes to allow a spouse or family member to benaefit from a full night’s sleep while their loved one is cared for.
